Mystery Modem

I need some help in identifying a PCI modem.
It has a speaker and line jack, and the info I can get off the modem is : FM-56PCI V:MTC
and an FCC number 552_5600-046.
There is a Motorola chip on the board itself.
Any idea what this is or where I can find any info?
